Overview of Programming & Services

A one page overview of programs & services offered through 1st Step Referral Services. Includes fees and a brief description. Case managers have found that providing a copy with the service required to enroll in checked on the form is helpful for client follow through.

Client Referral Form with Authorization

This form can be printed and faxed/mailed or e-mailed by selecting the SUBMIT button (for Adobe 8.2 and higher only). Includes client fees and contact information. When e-mailing, please indicate if you have a signed Authorization at your location.

BTC Calendar w/ Fees & Policies

This calendar is updated quarterly. When referring, it is beneficial to provide a copy to the client so they have all the information need before enrollment: BTC requirements, policies, fees, contact information and location.

Outpatient Brochure

This brochure can be given directly to the client. It outlines the basic programming structure covering the first 12 weeks of programming. Please contact our office for a complete overview of programming for up to 52 weeks.

We are providing this form to be used in conjunction with court requirements for Support Group Attendance. Feel free to print or forward to persons on your caseload.
