If you are seeking an evaluation for driver's license reinstatement through a state other than Michigan, it is imperative to fully understand all their requirements. Each state has a different process, different requirements, and different paperwork that will need to be submitted.

Be prepared before you begin the process to avoid frustration, wasting time and incurring additional expenses. Contact the department handling your case. You will need to request:

     1. An information packet that describes the components to be addressed
     2. All materials that need to be completed by you and your evaluator
     3. Contact information (address, fax number, phone number, and contact name, if           applicable) for returning the completed packet

     5. Driver's record, if applicable
     6. Timelines for submission
